What to Expect
We are so glad you have chosen the Eye Clinic of Wisconsin!
You can expect to be treated with courtesy, dignity, and respect. You have the right to informed consent in treatment decisions, timely access to specialty care, and confidentiality protections. You have the right to refuse treatment and to be informed of the medical consequences of your action.
On our website you can find helpful, appointment-related information, such as:

• Information on our hearing program and our optical department
Please bring the following items with you to your eye exam:
- Insurance Card(s)*
- A detailed list of any medications you are taking, either prescribed or over the counter
- Your glasses and contact lenses (if applicable)
- Sunglasses. Your eyes will be dilated
- Copay, due at check-in
- List of questions
* Insurance is a contract between YOU and YOUR insurance company.
BE INFORMED. Call your insurance company to review your benefits, including In-Network and Out-of-Network benefits, copays, deductibles, and out-of-pocket expenses. For Complete Eye Examinations, you will be asked to sign a form prior to the examination as to whether this visit is to be billed as routine or medical.
Other things you should know:
- Please allow 1-2 hours for a Complete Eye Exam.
- If you are coming in for a Contact Lens Exam, please wear your contacts for at least 2 hours prior to the exam.
- Your eyes will be dilated for the exam, this can last 4-24 hours. Dilation can make your close up vision blurry, cause some discomfort and light sensitivity. If you are unsure of how dilation affects you, consider having another person drive you home from the appointment.
- Please call 1-800-472-0033 to pre-register your insurance cards.
- Please have your records from your last eye doctor faxed to us at 715-261-8665.
- If the patient is under 18, please have a parent or legal guardian accompany them to the appointment. There are items that need to be signed by a parent or legal guardian at check-in. If a parent or legal guardian are unable to attend the appointment, please call the Eye Clinic of Wisconsin to make alternative arrangements for completing these documents and granting permission to someone else to make medical decisions for the patient during the exam.
